COLUMBUS, Ohio - The Ohio Department of Agriculture released its investigative summary Friday about the death at the Ohio State Fair on July 26.
Tyler Jarrell was killed when the Fireball ride came apart. He was a high school senior who just enlisted in the Marine Corps.
Seven other people were injured.
The department said while Amusements of America, the operators of the Fireball ride, notified the state immediately, the company failed to file a report within 24 hours as is required by law.
Despite this, the state chose not to fine Amusements of America or take any sanctions against the company.
A department spokesman said the company can still operate rides in Ohio starting next year - as long as they pass inspection and get the proper permits.
