Investigation Continues Into Fatal Elevator Accident

Authorities have shut down a second dormitory elevator following the death of Andrew Polakowski.kowski lost his life in an OSU dormitory elevator. The 18-year-old freshman died from asphyxiation after getting trapped between the elevator and the floor above.
 
So what exactly happened on that elevator? That's what investigators are trying to figure out.
 
The Columbus Fire Department says at the time of the elevator accident, the victim tried to pull himself up from the second floor to the third while halfway inside the elevator. What happened next remains under investigation.

By the time Columbus paramedics arrived at Stradley Hall, they estimate Polakowski was trapped for nearly seven minutes between an elevator and the second and third floor.
 
"He already had gotten his upper half of his body on his third floor and was trying to make the rest of the way out. Then what happened [is], the elevator started to descend again."

The weight would crush the student's chest, killing him. What we know about the tragedy is that the 1958 elevator was filled to capacity and likely over its weight limit at the time.
 
When the accident happened, the Columbus Fire Department says there were 24 people inside; 13 women and 11 men.

The floor of the elevator measured six feet by five and half feet, according to Ohio State.
 
The weight limit for that elevator is 2,500 pounds. Assuming each person weighed 140 pounds, the elevator's capacity was a thousand pounds over the limit.
