Lake Hallie nursing home is fined nearly $31,000

CHIPPEWA FALLS, Wis. (AP) — A Wisconsin nursing home has been barred from accepting new patients after investigators found it failed to do a proper background check, resulting in the hiring of a sex offender.

The Leader-Telegram reports ( ) the man worked as environmental services coordinator at Lake Hallie Memory Care in Lake Hallie. He was fired when company officials learned of his third-degree sexual assault conviction.

According to a report by the state Division of Quality Assurance, the man was hired after a background check was conducted improperly. He was fired Sept. 30.

The state fined the 77-bed center nearly $31,000 and ordered the ban after it discovered 30 total violations during an investigation from Sept. 24, 2013, to Jan. 9.

Nursing home operators sent its plan of correction to state officials March 11.
