RENO, Nev. (AP) — State health officials say the manufacturer of a steroid product suspected as the source of a deadly meningitis outbreak has confirmed five unused vials were the only ones shipped to Nevada and were recovered this week.

Nevada Department of Health and Human Services spokeswoman Mary Woods said Friday the steroid maker determined it shipped five vials to just one clinic in southern Nevada in July. The name of the clinic hasn't been released.

Southern Nevada Health District spokeswoman Stephanie Bethel said the vials were recovered by the U.S. Food and Drug Administration.

The rare form of fungal meningitis is being blamed for at least five deaths and 35 illnesses in six states. The Massachusetts-based pharmacy involved has recalled more than 17,000 single-dose vials of the steroid used for back pain.
