The Lost Shepherd closed his week after the Delaware Board of Health revoked its food service license for improper procedures.
According to the Delaware General Health District, a Nov. 8 investigation revealed critical food-safety violations involving improper food cooling procedures, cold foods not kept cold enough and employees failing to wash hands as required.
The restaurant, located at 345 W. Olentangy St. in Powell, opened in 2008, 10TV's Kevin Landers reported.
Since it opened, the Board of Health inspected the tavern 23 times and documented 51 critical food safety violations, including finding foods at unsafe temperatures and food thawing in a sink designated for employee hand washing.
Another investigation showed that The Lost Shepherd did not have food thermometers, 10TV News reported.
According to the Delaware General Health District, the restaurant owner was to two administrative hearings in October 2010 and August 2011, and the owner agreed to take steps towards increased food safety.
Landers reported that a new owner plans to reopen the restaurant next week under a new name.
Joan Sengewalt, who lives across the street from the restaurant, said she and her family would go to the restaurant every week.
"We go every Wednesday night," she said. "(You could get) the fish and chips and a beer for five dollars."
Sengewalt said she would go to the new restaurant when it opened and never had a bad experience at The Lost Shepherd, Landers reported.
